CONTESTANT AGREEMENT

Contestant's First and Last Name 

Contestant confirms that he/she meets the following four (4) Requirements:

       a)      Contestant is at least 18 years of age.

b)      Contestant has never been convicted of a crime in the United States or elsewhere.

 c)      Contestant is physically capable of engaging in the MovieStar competition.

d)      Contestant is mentally competent.

Furthermore, Contestant agrees individually, and on behalf of his/her heirs, representatives, agents, successors, executors, administrators and assignees, to the following five (5) Terms and Conditions, regarding the acting competition and television show, MovieStar.

a)      Contestant consents, authorizes and permits MovieStar to use Contestant’s likeness

      and image in its promotion, development, production, advertising, and distribution

      without compensation.

b)      Contestant agrees that MovieStar shall own the copyrights, performance rights, trademarks, and all other legal rights to all audio, video, and other recordings or images of Contestant created during the course of production.

c)      MovieStar may use Contestant’s likeness and image for all purposes, including but not limited to television broadcasts, advertisements, and promotions.

d)      MovieStar reserves the right to disqualify participants who engage or have engaged in illegal activity.

e)      Contestant agrees to waive any and all claims (compensatory, direct, incidental, consequential, or otherwise) against MovieStar, its creators, producers, sponsors, employees, agents, assignees, and successors, for any and all claims arising during the development, production, or distribution of MovieStar.
